Introduction

<p>Attested copies of the following documents (or in original where mentioned) must be attached with the form. These documents are not returned to the candidates in case they are admitted.</p> <p>For 1st year paste one passport size blue backround latest photo on the front of online application form.</p> <p>Provisional certificate from the last institute attended.</p> <p>Character Certificate issued by the institution last attended. Where the candidate has passed the last examination in private capacity such certificate shall be signed by a gazetted officer.</p> <p>Detail Marks Certificate (DMC) of the last examination taken. Candidates who passed the last examination as a private student must also submit their original DMC.</p> <p>Self / father(guardian) CNIC / Form B.</p> <p>Original affidavit in light of Supreme Court decision for refraining from politics on a stamp paper of Rs.20. Copy of the affidavit is attached with the prospectus for ready reference.</p> <p>For admission in 1st year, Migration certificate in original, in case the candidate has passed the exam other than BISE Abbottabad.</p> <p>For admission in BS the Migration certificate, in original, from board to University of Haripur must be submitted.</p> <p>Candidate seeking admission in BS should submit the copy of original SSC from the board.</p> <p>Four recent passport size photographs.</p> <p>For Bs paste one passport size white&nbsp;backround latest photo on the front of online application form</p> <p>Domicile Certificate.</p> <p>Age Relaxation Certificate by the competent authority should be tendered in case the age of the candidate exceeds the prescribed upper age limit.</p> <p>​Original Documents to be produced at the time of interview SSC / FA / FSc Provisional Certificate SSC / FA / FSc DMC Character Certificate from the head of the institution last attended. Domicile Certificate Father&amp;amp;amp;Guardian and Student CNIC where applicable.</p>
