• DMC of last exam passed.( 2 copies)
• Character Certificate
• Form-B
• Father CNIC & Mother CNIC
• Five  passport size picture
• Online application form in hard form.
• Sports certificate (for sports quota)
• Disability certificate (for disable quota)
• Residence certificate
• Parents employment certificate from HED (for HED employees only)
• Migration certificate(Other Board)
• Stamp paper from court for admitting that he had not taken admission in any other college ( Affidavit)
• Afghan (approval of documents from afghan commissioner)
