Instruction:

The application form of the online apply along with the attested copies of following documents should be provided at time of admission

1.     SSC /  Matric Detailed Marks Certificate (DMC) 
2.      Detailed Marks Certificate (DMC) of HSSC
3.      HSSC/DAE Certificate for admission at BS-Level
4.      Provisional Certificate of HSSC/DAE
5.      Character Certificate issued by the learning institution last attended
6.       Domicile Certificate
7.       Self / father CNIC / Form B
8.       Six (06) passport size coloured photographs.
9.        Migration Certificate for candidates having passed their examination from other than BISE Mardan/ University of Swabi.
10.       Quota eligibility certificate in case a candidate applies for a specified quota notified by the government, e.g Sports, Disable or Higher Education Employees Khyber Pakhtunkhwa(college subsector) quotas, etc. 
11.        Affidavit as per specimen.

        Note: Migration certificate- and affidavit in light of Supreme Court decision for refraining from politics-(i) will be obtained from the candidates at the time when their admission is confirmed.
