Introduction

Attested copies of the following documents (or in original where mentioned) must be attached with the form. These documents are not returned to the candidates in case they are admitted.

Provisional certificate from the last institute attended. ( 2 copies)
Character Certificate issued by the institution last attended. Where the candidate has passed the last examination in private capacity such certificate shall be signed by a gazetted officer. ( 2 copies)
Detail Marks Certificate (DMC) of the last examination taken. Candidates who passed the last examination as a private student must also submit their original DMC. ( 2 copies)
Self / father(guardian) CNIC / Form B ( 2 copies)
Affidavit as per specimen.
.For admission in 1st year, Migration certificate in original, in case the candidate has passed the exam other than BISE Abbottabad.
For admission in third year the Migration certificate, in original, from board to University of Haripur must be submitted.
Candidate seeking admission in third year should submit the copy of original SSC from the board.
Four recent passport size photographs.
Domicile Certificate.
Original Documents to be produced at the time of interview
1. SSC / FA / FSc Provisional Certificate
2. SSC / FA / FSc DMC
3. Character Certificate from the head of the institution last attended
4. Domicile Certificate
5. Father’s / Guardian’s and Student’s NIC where applicable
