  1. Copy of Admission Form
  2. SSC Certificate
  3. SSC DMC
  4. Character Certificate
  5. Domicile
  6. Undertaking as per Specimen
  7. Selft CNIC or B-Form
  8. Father CNIC
  9. Colored Photograph (4)
  10. Quota Eligibility Certificate, in case candiate applied for specific Quota.e.g Sports, Diable, HED Employee
  11. Migration Certificate if exam passed other than BISE Kohat
